Vacancy Hours: 37

Contract Type: Permanent

Salary: Up to £30,451

Weekend Working: Yes

*** Welcoming individuals looking to relocate / relocation allowance available***

Are you looking for a new opportunity in the world of social care?

We work in adult social care to make a difference and change lives – join us and do exactly that! We have a number of Social Care Coordinator opportunities working for Harrogate and Rural Alliance (HARA) fulltime, on a full time basis.

The role offers a blended approach to working, with some days spent working at home and some spent locally.

Whilst work can be undertaken from your home base, there will be a requirement to attend the office on occasions due to service need. While virtual working is promoted as a modern and efficient way of delivering services, the need for face-to-face visits will also be required at times, depending on the needs and circumstances of the individual you are working with. With this flexibility in mind, we encourage applicants to apply who might wish to work more flexibly or alternative hours ( for example only weekends).

HARA brings together NHS Commissioners, service providers and North Yorkshire County Council to work together to improve how community health and social care is provided for adults in the Harrogate District. From the 30th September 2019 the Harrogate District became one of the first places in England where community health and social care service were linked to local Primary Care practices with community nurses, therapists, and social care practitioners working together to respond to people's needs.

We are proud of our successful track record as a leading care service provider, and consistently appear amongst the top performing County Councils in the country. There’s no better time to start a career with North Yorkshire County Council - the time for change and innovative approaches to delivering care is something you could be part of!

As a Social Care Coordinator, you will carry out reviews and assessments as required, working within a supportive and forward thinking team within the Harrogate and Rural area, making decisions about eligibility as part of the Care Act 2014 and develop creative solutions with people in response to those reviews and assessments. Where people do not meet eligibility you will be expected to signpost people to other relevant services and support, and so an insight into local resources will be an advantage.

We are looking for someone with significant experience of social care assessments in order to undertake Care Act reviews with adults and older people within a local authority setting. You will be able to respond quickly to urgent and changing situations to minimise risk to people with care and support needs whilst respecting people’s choices and raising concerns through safeguarding. You will also demonstrate the importance of dignity and respect in all aspects of the role.

To be eligible for the Social Care Coordinator post an NVQ 3/level 3 Diploma in Health and Social Care (or equivalent) is essential.

In return we can offer you:

  • The opportunity to develop knowledge, experience and skills with a leading care provider
  • Professional support and supervision from your nominated team manager and relevant training
  • The challenge of complex and varied work through a manageable caseload
  • The chance to be part of a committed and supportive team
  • Access a range of benefits including generous leave entitlement, flexible working, a local government pension scheme, salary sacrifice and employee discount benefits
  • Access health and well-being advice and support for you and your family
  • The ability to utilise new technology enabling modern ways of working and supportive flexible working

If you would like to be at the forefront of innovative social care practice, we would like to hear from you.

For an informal discussion about the post (which we could encourage prior to application) please contact Sue Jenkinson on Sue.2.Jenkinson@northyorks.gov.uk
