We are looking for an innovative Social Worker with 2 years post qualifying experience to join our Transforming Care Team.

This role is completely home based, allowing candidates flexibility regarding their location and work patterns whilst ensuring maximum productivity and efficiency.

Whilst the role within the transforming care team is initially fixed term until end of March 2022, you will be employed on a permanent contract and we will work with you to find you an alternative social worker role within NYCC beyond March 22.

Working closely with our colleagues in health, our Transforming Care Team support people with a formal diagnosis of a Learning Disability, Autism or both who are currently detained under the mental health act or at risk of being detained under the mental health act.

Our team play a fundamental role in improving the lives of people with a learning disability, autism or both to enable them to live in the community they choose to live in with the right support. We support the prevention of people being detained under the mental health act, which leads to a hospital admission, and support people to be discharged from hospital who are currently detained under the mental health act.

  

You would be joining us at a pivotal time for both North Yorkshire and the County Council's development as we move towards our 2025 plan and working through the current COVID pandemic.

As a Social Worker you will support the team in the delivery of the service, through contributing to practice and performance improvement. You will be responsible for working with people to meet their outcomes to become or stay part of their community and maintain/improve the person’s independence in daily living activities.

During these times the Transforming Care Team are working remotely from home. All technology required to enable this will be provided. NYCC also offers a comprehensive virtual induction programme.

As stipulated in the amended  Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, post holders must demonstrate evidence of having had a complete course of an authorised COVID-19 vaccine.

If you haven’t had a full course of the vaccination at the moment, you can still apply, but in line with new legislation you must have received both doses prior to your start date.  Alternatively, if you are medically exempt from receiving the vaccine, you can currently provide a self-declaration, but must have applied and received formal clinical exemption via the COVID Pass Route by 24 December 2021
