We currently looking for a qualified Social Worker to join our team in the Discharge Hub at Scarborough Hospital to help us realise our ambition of ensuring that all residents of North Yorkshire live longer, healthier, independent lives.

Whether you are a newly qualified social worker looking for your first opportunity or whether you are an experienced social worker looking for a new challenge, this could be the role for you!

This unique opportunity in the Discharge Hub is a full time, established post. Based in Scarborough our team predominantly covers hospital discharges emanating from Scarborough hospital sites including Whitby, Matlon and Bridlington. This role offers a blended approach to home and on-site working.

You will undertake assessments under Discharge to Assess guidelines whilst being mindful of the importance of a strengths-based focus and preventative measures to ensure we have the person’s wellbeing at the heart of what we do. Mental capacity assessments and best interest decision making and s117 assessment and care planning of people being discharged from hospital and safeguarding vulnerable adults will be part of your day to day work. We can be creative with the approaches that we use to ensuring that people have their needs met in a safe manner, whilst clearly noting the importance of thinking ‘Home First’.

There’s no better time to start a career with us- the time for change and innovative approaches to delivering care is something you could be part of! We are fortunate to be able to invest in new professional leadership in Social Work, as well as developing wider career paths for staff. We fully recognise the need to support staff development and enhance their skills and experience, which we deliver through senior professionals within the service who can share the wealth of experience they have gained through years as social care practitioners.

We know Adult Social work is a challenging profession. Therefore, it is important to us that our social workers have manageable workloads, supportive and transparent management and the opportunity for continuous professional development. We excel in giving our colleagues commitment and high levels of support, enabling them to become the best social workers they can be. We are committed to creating a healthy culture where our social workers feel able to share their views, to achieve work/life balance and to continually learn and develop new ways of working. As an organisation, we promote a safe culture of support and learning which we expect all staff to endorse.

We recognise the importance of the health and wellbeing of our employees, and provide them with specialised administrative support, allowing focus on delivery of care services. We also support the AYSE (Assessed and Supported Year of Employment) programme for newly qualified Social Workers where you will be supported in your first full year of employment through reduced caseloads and enhanced supervision.
