We are currently looking for a social worker to help us realise our ambition of ensuring that all residents of North Yorkshire live longer, healthier, independent lives.

This unique opportunity is a full time, established post in our Discharge Hub Team. Based in Northallerton our team predominantly covers hospital discharges emanating from the South Tees Hospital Trust sites including James Cook, Friarage, and the Friary. We do also support Darlington Memorial Hospital, Bishop Auckland General Hospital and other outlying hospitals too with people who are being discharged from hospital living in the northern patch of North Yorkshire.

Working in a fast-paced environment, you will work closely with our health colleagues to ensure that seamless pathways of care are delivered under discharge to assess arrangements. You will undertake assessments under discharge to assess arrangements whilst being mindful of the importance of a strengths-based focus and preventative measures to ensure we have the person’s wellbeing at the heart of what we do. You may need to be involved in considering the Mental Capacity of people being discharged from hospital, if this is called into question and requires Best Interests decisions to be made on their behalf involving both families, health professionals and other relevant partners in our wider communities. We can be creative with the approaches that we use to ensuring that someone is able to have their needs met in a safe manner, whilst clearly noting the importance of thinking ‘Home First’.

At the same, you will be considering whether there are any informal carers who may need to be referred for assessments in their own right. Within your work, you will also have key responsibilities for ensuring that vulnerable adults are safe from harm but where harm or abuse is identified for ensuring relevant safeguarding enquiries are progressed.

To be eligible for this post you will have a professional Social Work Qualification, DipSW or CQSW and you will have current registration with Social Work England. An Enhanced Level DBS Disclosure is required for appointment to this post.
