As part of its wider prevention program North Yorkshire County Council has invested in a Living Well team. The Living Well Coordinators work with individuals and their Carers.  They enable them to remain living independently by helping people access their local community and supporting them to improve their health and wellbeing. The purpose of this role is to help to reduce loneliness and isolation and to prevent or resolve issues for people before they become a crisis.

 

The Living Well Coordinators help people to make small improvements in their lives to improve their health, well-being and independence. They achieve this by helping people to increase their confidence, develop links into their local community and offering access to good quality advice and information.

 

As well as working with individuals the Living Well Coordinators work closely with teams within the council such as Public Health and Stronger Communities. They also work in partnerships with other organisations e.g. Voluntary, and Community Sector (VCS).Health Colleagues, NHS Clinical Commissioning Groups and District Councils.

 

The role requires excellent communication and interpersonal skills and the successful candidate will be able to demonstrate the ability to motivate people and engage with them in a non-judgmental way, This is a challenging role but offers the opportunity to make a real difference to people’s lives.
