Team Manager – Discharge Hub  

 

Are you an aspiring manager looking for a new challenge within care and support?  We have an exciting opportunities for a Team Manager in the Scarborough Discharge Hub.   If you get satisfaction from working in a busy multi-disciplinary environment we would love to hear from you. 

You will work alongside health colleagues and lead on the Discharge to Access Pathway.

You will manage a team of Social Workers and transfer of care coordinators working with the trusted assessment. You will deliver on the key principles of home first, safe and timely discharges across a number of hospitals. 

You will support the Service Manager in the delivery of services, through supervision of staff, responsibility for delegated aspects of budget management and contributing to practice and performance improvements. Managing specific portfolios, you will develop and distribute specialist practice, process and performance knowledge. You will ensure the services delivered are effective, considering risk and best value. You will be working alongside our community teams, our Reablement Service and Occupational Therapy Team. You will also work in partnership with our Health colleagues in Primary and Secondary care.

This role offers opportunities for a blended approach to working, with the ability to work from the office and home.  There will be a requirement to attend the work base on occasions due to service need.

You will require a professional Social Work Qualification and a current registration with Social Work England. You will also require an enhanced DBS and evidence of up to date Covid Vaccinations.
