Salary: Up to £30,451 per annum

Hours: 37 per week

Contract: Permanent

Following a successful pilot, we are now looking to establish a permanent team who will support Continuing Healthcare assessments, in partnership with our Health colleagues.

 

Our roles allow a flexible approach to working. Whilst work can be undertaken from your home base, there will be a requirement to attend the office on occasions due to service need. While virtual working is promoted as a modern and efficient way of delivering services, the need for face-to-face visits will also be required at times, depending on the needs and circumstances of the individual you are working with. With this flexibility in mind, we encourage applicants to apply who might wish to work more flexibly or alternative hours ( for example only weekends).

 

The team will be managed by a dedicated team manager, and as we recognise the value of multi-professional teams, will include roles such as; Social Workers, including Mental Health Social Workers, Social Care Coordinators and Occupational Therapists.

 

You will be instrumental in supporting improved outcomes for people who require assessments for Continuing Healthcare. As a worker in this team, you will support all assessments and decision making regarding Continuing Healthcare assessments. You will case manage a caseload where there are joint package arrangements between social care and health, completing timely reviews to ensure the persons needs and outcomes are met appropriately.

 

You will have become a specialist in the field of Continuing Healthcare, supporting people and their families through the assessment process and sharing your skills and knowledge with the wider Health and Adults Services Directorate.

 

We would love to hear from you if you have experience of assessments under the Care Act 2014, in the completion of NHS CHC Checklist tool, and being part of a Multi-disciplinary team to complete the Decision Support Tool. We would welcome applications from assessors with specific experience in supporting people with complex mental health issues or people with learning disabilities, although this is not essential to the position. We would also specifically value Occupational Therapy within the team and the unique perspective and dual social care & health training that the profession brings.

 

We value the specialist knowledge this team has, and we support the team with daily catch ups and specific practice sessions so there are lots of opportunities to support your continued professional development as well as to ensure you have peers available to seek advice and support from.

We have established strong relationships with our NHS partners and we work jointly to create the right pathways and processes for the team.

 

The team is demonstrating a real difference, and we have shown we can improve the outcomes for people who receive our support. The team will continue to have a key role in the development and shaping the future for the CHC Service within NYCC.

The team works alongside our NHS colleagues Monday to Thursday  08.30 to 17:00 & Friday 08:30 to 16:30, and we offer flexibility within the hours of work which can be discussed and considered.

At present the team is not required to work weekends but this may be subject to change in the future should the service require it to do so.

 

This is a pivotal supporting role in North Yorkshire, and we have various offices available throughout North Yorkshire- so wherever you are currently located, we can work with you to organise the most practical office base for you. We are also currently working remotely, and can offer a variety or remote working and office working as offices in North Yorkshire begin to reopen.

As stipulated in the amended  Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, post holders must demonstrate evidence of having had a complete course of an authorised COVID-19 vaccine.
