We are currently looking for a social care coordinator to join our team in the Whitby area on a full time, fixed term basis.

The role offers a blended approach to working, with some days spent working from home and other days spent locally.

As a Social Care Coordinator you will carry out reviews and assessments as required, working within a supportive and forward thinking team in Whitby, making decisions about eligibility as part of the Care Act 2014 and revising and developing creative solutions with people in response to those reviews and assessments. Where people do not meet eligibility you will be expected to signpost people to other relevant services and support, and so an insight into local resources will be an advantage. You will focus primarily on undertaking reviews with people with care and support needs who are currently receiving support either at home or in a supported setting. 

We are looking for someone with experience of social care assessments, and Care Act reviews with adults and older people within a local authority setting. You will be able to respond quickly to urgent and changing situations to minimise risk to people with care and support needs whilst respecting people’s choices and raising concerns through safeguarding as indicated. You will also demonstrate the importance of dignity and respect in all aspects of the role. To be eligible for the social care coordinator post an NVQ 3/level 3 Diploma in Health and Social Care (or equivalent) is essential. There is an expectation that the successful applicant will be able travel across the locality.

Working from Whitby, your office base is on the South side of town with on-site parking and easy access to the A171. The team works across the area from Staithes down to Robin Hoods Bay and across to Great Fryupdale. The area covers one of the largest expanses of heather moorland in the United Kingdom with villages hamlets and towns reaching down to the world famous North Yorkshire and Cleveland Heritage coast. There is some flexibility to occasionally work from the NYCC office in Scarborough as well as from home.

As stipulated in  Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 it is mandated that post holders must demonstrate evidence of having had a complete course of an authorised COVID-19 vaccine.
