We are currently looking for qualified social workers to help us realise our ambition of ensuring that all residents of North Yorkshire live longer, healthier, independent lives.

We have established a new community peripatetic social care team to work across the Vale of York area. The Vale of York geography covers Ryedale (Malton, Pickering and Helmsley), Hambleton South (Thirsk and Easingwold) and Selby. The role offers a blended approach to working from home and in the community, with occasional weekend cover. Whilst work can be undertaken from your home base, there will be a requirement to attend the office on occasions due to service need. While virtual working is promoted as a modern and efficient way of delivering services, the need for face-to-face visits will also be required at times, depending on the needs and circumstances of the individual you are working with. With this flexibility in mind, we encourage applicants to apply who might wish to work more flexibly or alternative hours (for example only weekends).
The team works with adults from the age of 18, and includes working with people with physical disabilities, autism, learning disabilities and other long-term conditions and frailty. We also work with younger adults transitioning in to adult’s services.
The role will entail working with people under s117’s aftercare, Mental Capacity Assessments, Care Act assessments, safeguarding adults as well supporting people to live independently in a wide range of settings in the community such as supported living and extra care housing.
As a Social Worker, forming collaborative partnerships will be key as you will be working closely with internal stakeholders such as Stronger Communities and Living Well and external stakeholders including Health colleagues, Police and the voluntary and independent sectors.
Through strength based assessments and interventions there will be a focus on holistic, person centred care, supporting adults with care and support needs and their carers to achieve their desired outcomes and wellbeing.

To be eligible for this post you will have a professional Social Work Qualification, DipSW or CQSW and you will have current registration with Social Work England.
An Enhanced Level DBS Disclosure is required for appointment to this post.

evidence of having had a complete course of an authorised COVID-19 vaccine.
