At this very exciting time we are looking for a Mental Health Social Worker to join us, based within both Gibraltar House and also White Rose House in Northallerton and help us realise this vision. As a mental health social worker you will have responsibility for assessing, planning, implementing and evaluating social care for individuals in a community setting. You will support people within the secondary care pathway of the team, using a recovery model approach to help maximise people's potential to maintain their dignity and independence in their own homes and communities. You will provide professional support in terms of interventions with individuals, families and groups and jointly work and communicate with staff across the primary and secondary care interface. You will also liaise with non-statutory organisations to meet service user /carer needs. Working in partnership with members of the multi-disciplinary team e.g. medical staff, occupational therapists, psychologist, social care staff, nursing care staff and students on placement, you will deliver the requirements of the Care Act and other statutory duties such as Adult Safeguarding.

To be eligible for this post you will have a professional Social Care qualification such as Social Work, DipSW or CQSW and have current registration with the Health and Care Professions Council In return we can:

Enhanced Level DBS Disclose is required for appointment to this post.
