We are currently looking for a social care coordinator to join our team in the Vale of York area on a full time, permanent basis.

This role offers a blended approach to working, with 3 days a week dedicated to home working and 2 days a week spent locally, within a 7 day working week.

 

These roles are based within our Vale of York community team which covers Ryedale, Hambleton South and Selby and more specifically ,Malton, Pickering, Helmsley, Selby Thirsk and Easingwold . This team manages social care interventions and services based within the community- received from Care and Support or Discharge Hubs. Community Social Care teams are generic adult social care teams, working with people with physical disabilities, autism, learning disabilities and other long term conditions and frailty. We also work with young people transitioning to adult’s services

 

As social care coordinator, you will be responsible for undertaking a range of asset and strength based assessments and interventions to ensure that adults with care and support needs and carers achieve their desired outcomes and wellbeing.  You will determine eligibility for adult social care service set against the national standard, working with adults with care and support needs and carers to develop personalised care and support plans that represent best value and ensuring that adults with care and support needs and carers are safe from harm.

 

To be eligible for the social care coordinator post an NVQ 3/level 3 Diploma in Health and Social Care (or equivalent) is essential. An Enhanced Level DBS Disclosure is required for appointment to this post.

As stipulated in  Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 it is mandated that post holders must demonstrate evidence of having had a complete course of an authorised COVID-19 vaccine.

If you haven’t had a full course of the vaccination at the moment, you can still apply, but in line with new legislation you will need to be committed to and show evidence of your first dose by the 16th September 2021 and your second dose by the 11th November 2021, employment beyond this date cannot continue without appropriate COVID-19 vaccination.
