Are you looking for a new opportunity in the world of social work?

Why not be part of an innovative, award-winning social work team?

We work in adult social care to make a difference and change lives – join us and do exactly that!

We are looking to recruit a Social Care Coordinator to work with the Adult Community Social Care Team in the Selby area on a full time, fixed term basis.

This role offers a blended approach to home and on-site working, with some days spent working from home and others spent locally.

As a Social Care Coordinator you will carry out reviews and assessments as required, working within a supportive and forward thinking team in Craven, making decisions about eligibility as part of the Care Act 2014 and revising and developing creative solutions with people in response to those reviews and assessments. Where people do not meet eligibility you will be expected to signpost people to other relevant services and support, and so an insight into local resources will be an advantage. You will focus primarily on undertaking reviews with people with care and support needs who are currently receiving support either at home or in a supported setting.

You will be able to respond quickly to urgent and changing situations to minimize risk to an individual's safety and wellbeing, whilst respecting the person's choices and be involved in raising concerns through safeguarding when required. You will demonstrate the importance of dignity and respect in all aspects of the role.

As an organisation, we promote a safe culture of support and learning and strive to ensure our staff have manageable case workloads, providing a robust induction focusing on the delivery of care services.

 We can also offer you:

•The opportunity to develop knowledge, experience and skills with a leading care provider
• Professional support and supervision from your nominated team manager and relevant training
•The challenge of complex and varied work through a manageable caseload
•Opportunity to work in different specialisms
•The chance to be part of a committed and supportive team
• Access a range of benefits including generous leave entitlement, flexible working, a local government pension scheme, salary sacrifice and employee discount benefits
• Access health and well-being advice and support for you and your family
• The ability to utilise new technology enabling modern ways of working and supportive flexible working.

To be eligible for this post an NVQ 3/level 3 Diploma in Health and Social Care (or equivalent) is essential. There is an expectation that the successful applicant will be able travel across the locality.

We would encourage you to have an informal conversation with the recruiting manager prior to applying.  Please contact: Sue Jenkinson on 01609 798089.
