We have an exciting opportunity for a Team Manager to manage a well-established community team in the Vale of York area  which covers Ryedale, Hambleton South and Selby  and more specifically, Malton, Pickering, Helmsley, Selby, Thirsk and Easingwold.

  

As team manager you will have a passion for promoting strength-based, person-centred approaches that promote and maximise independence and wellbeing. The assessments completed by the team are to be asset and strength based ensuring independence is maximised whilst proactively developing innovative solutions to meet the requirements of adults with care and support needs and their carers.

 

You will manage a team of professionally qualified Social Workers and vocationally  trained staff and will be directly responsible for supporting the Service Manager in the delivery of services, through supervision of staff, responsibility for delegated aspects of budget management and contributing to practice and performance improvements. Managing specific portfolios, you will develop and distribute specialist practice, process and performance knowledge.

 

You would be joining us at a pivotal time as we have restructured our Health and Adult Services in line with the Hospital Discharge to Assess Policy.  You will be working alongside four other Community Support teams covering the Vale of York, a Hospital Hub Team in York and our Reablement Service and Occupational Therapy Team.  We also offer a preventive model with an excellent Living Well Service and provide Front Door advice and information.

This post is intended to cover a wider patch initially to support resilience across the Vale of York. The Vale of York geography will be reviewed over the next 12 to 18 months and this post is likely to move into a specific locality for community social care team management following that review.

 

You will require a professional social care or health qualification, for example, Social Work or OT with current registration with relevant professional body. You will also require an enhanced DBS.
